Program Description

The Community Care Management Program works closely with health plans to identify members for a community-based program with access to a full continuum of mental health and substance abuse services through the CHIPA network of contracted providers. 

The primary goal of the program is for providers to complete face-to-face community based assessments of the identified population to determine the most clinically appropriate and cost­-effective therapeutic services by identifying the risks, needs and barriers that are impeding their access to effective treatment. We want to ensure that the identified CCM eligible members receive timely access to clinically appropriate medical and behavioral health care services, the health plan, and Beacon CBHM believe that quality clinical services can achieve improved outcomes for our members.  CCM combines traditional case management with “feet-on-the-street” assessment, engagement, and advocacy. 

The responsibility of the CCM program are as follows:

  • Face-to-face assessments and care coordination

  • Telephonic coaching

  • Care coordination between medical and behavioral health providers

  • Community resource referrals

  • Health advocacy

While College Health IPA is the contracted service provider, management of the CCM program, including clinical oversight, quality management, and claims payment is provided by Beacon CBHM, a Beacon Health Holdings (Beacon) company.

Presently, the Beacon companies (Beacon CBHM, Beacon Health Strategies, and Psychcare) provide care management services to 6.2 million members through its partnerships with client Plans and care management organizations.  Most often co-located at the physical location of our Plan partners, Beacon’s “in-sourced” approach deploys utilization managers, case managers, and provider network professionals into each local market where Beacon conducts business.  Working closely with our Plan partner, this approach facilitates better coordination of care for members with physical, behavioral, and social conditions and is designed to support a “medical home” model. Quantifiable results prove that this approach improves the lives of individuals and their families and helps Plans to better integrate behavioral health with medical health.
